I can't answer for Bob & Jo-Ann, but it's a pretty simple thing for me -- since Ephesians 5:22 clearly states that wives should submit to their husbands, I think that's the answer to the question "in a marriage."  I think verse 21 is in a different context, with Christians in general, and is the end of a separate paragraph.

You probably identified a book enhancement to reduce the range of answers from multiple down to one. My old T&T book design looked like there was an inadvertent ambiguity between two verses, 21 and 22. The question was labeled for verse 21, but phrased for verse 22. The answer might be made simpler for the clubber if the leader picks one of the verses and adjusts the question for that one verse. The question might be made easier for AWANA leaders and clubbers by asking the publishers about reducing the range of answers in a future book revision. Good job paying attention to the book's configuration.
